For your convenience, an online client information form has been provided. This form will need to be completed prior to your first session with Patty Mohler Counseling. The form is submitted over a secured network. Your confidentiality is my utmost concern. If you are not comfortable with sending the information, simply download the form and bring it on your first visit.
Also available for download are the HIPPA Form and a Professional Disclosure Statement. Both forms will need to be reviewed, signed and brought with you for your first visit. I look forward to meeting you!
